ITD-0280, Feasibility Study	This form has replaced the ITD-0651, Preliminary Environmental Evaluation form as of January, 2007.

ITD-771, Professional Agreement Invoice & Progress Report	Consultants are required to submit this form monthly to the Agreement Administrator regardless if an invoice is submitted.

ITD-2892, Certification of Payment	Consultants are required to fill out this form every month certifying that they paid their sub-consultants within 20 days of receipt of payment from the Sponsor or ITD. This form must be submitted with each invoice.

Affidavit of Indebtedness	This is the affidavit of indebtedness that is required to be signed by the Consultant prior to the Sponsor's final payment for services.

Certified Wage Rate format	This is the format that is required to be used for certified hourly wage rates. The certified hourly wage rates, in pdf format, should be emailed to Mike Moffett or Jerry Flatz AND Holly McClure at ITD.
